Payment Policy

Payment is due in full the day of service, including co-payments, co-insurance and any other charges. We will be happy to file a claim for your care with your insurance carrier, however some plans have high deductible amounts and patients are responsible for payment of charges until the deductible is met. If you are having financial difficulties paying your medical expenses please contact our billing department.

We accept cash, check and all major credit cards for payment of services.

Insurance

Please bring your insurance information and necessary forms with you at the time of your appointment. Dr. Patel participates with most commercial and market place plans, we ask that you please check with your insurance company see if we participate with your plan. It is best to check with your insurance to see if your plan requires a prior-authorization or referral before services.
